Once you are scheduled for your appointment, you'll receive an email confirmation with links to these forms. If that goes missing, you can always circle back around here to fill out forms. Maybe not the fun part of working with a new practitioner, but it is essential for running my business in a way that keeps everyone on the same page and also, provides me the information I need to serve you in the best way possible!
1. Please read & sign the Client Policy Agreement.
2. Please take 5-10 minutes to fill out the appropriate intake before your session. This will help prepare me for our time together.

Life happens and a Cancelation Policy is necessary for my little, small biz.
I have a very limited number of sessions available to clients these days (because of my web design business...so fun!) so each spot is a highly-prized little spot! If you cancel or reschedule with as much lead time as possible, that'd be great; I can get someone else into your spot if you give me enough time to make that happen.
24 HOURS POLICY
If you cancel less than 24 hours in advance of your scheduled session, you will be charged 50% of the appointment cost. If you cancel the same day, you will be charged 100% of your appointment cost.
(Emergencies will be considered, of course.)
NO SHOW POLICY
Clients who fail to show for an appointment will be charged 100% of the scheduled service fee and may be asked to pre-pay for future services.
LATE ARRIVAL POLICY
Should you arrive later than your appointment time you will likely have a shortened session so that I can keep on schedule with other clients. If time permits, you may still receive your full session.
If you are more than 15 minutes late and have not notified me that you are on your way, I reserve the right to handle this as a No Show and may leave my office. You will be charged as a No Show, 100% of your scheduled session fee.
Should an emergency situation arise, please let me know ASAP that you need to cancel your appointment. Rarely does it happen, but I reserve the right to charge 100% of the scheduled service fee. If this sort of cancellation happens more than two times within six months, a client may be asked to pay in advance for future sessions.
